For the purposes of marketing, advertising, promotional and/or communication purposes, the Lemonade Collective may, from time to time, take photographs and/or video recordings of Lemonade Collective based activities or events that include the Lemonade Collective members/participants.

By completing this Form, you are consenting to the taking of photographs and/or video recordings of your child by the Lemonade Collective staff and/or contract instructors, and waive any rights you have related to, any such photographs and/or video recordings, and you are consenting to the use of any such photographs and/or video recordings, in whole or in part, by the Lemonade Collective.
